Chart Connect

Chart Connect is an online physician portal that allows referring and external clinicians to securely access their patients’ MU Health Care electronic medical records, including clinic notes, lab results, radiology notes and other information. Authorized users can review patient records in real-time, 24/7.

Eligibility Requirements:

  • Physicians and Advanced Practice Providers: Must have an active Chart Connect account or be linked to a sponsor with an active account.
  • Support Staff: Must be assigned access by a sponsoring provider before completing the request form.

How to Access Chart Connect

Step 1: Request to Apply

  1. Complete a request for an application here.
  2. This form is only to request an application and will not automatically grant access to Chart Connect.
  3. Upon submission, you will receive an application within 72 hours.

Helpful Hints:

  • All communication regarding your Chart Connect account will be sent to the email address you provide.
  • Support Staff: Select the title of “other” on the form and list your sponsoring clinician in the Approved Sponsor section.

Step 2: Complete the Application

  1. You will receive an email asking you to set up a username and password for the application site.
  2. Once your username and password are established, you may fill out your application.
  3. The application consists of 8 sections:
    • Demographics: Information about the user applying.
    • Office/Location: The user’s clinic or work location address.
    • Licensure/Certification: Medical license information (Support Staff without licenses can skip this section).
    • Approved Associates: Support staff must list their Approved Sponsor (sponsoring clinician). Similarly, clinicians should list their support staff.
    • Confidentiality Agreement: Read and click Agree.
    • HIPAA Attestation: Read and click Agree.
    • HIPAA Training: Read and click Agree.
    • Documents and Images: Sign and date after confirming application accuracy.

Helpful Hint: Ensure the application shows 8 of 8 Sections Completed 100% before submission.

Step 3: Set Up Your Account and Gain Access

  1. Your application will be reviewed, and you will receive an email once approved.
  2. That email will contain details about how to log on to Chart Connect, where you will create a username and password specifically for Chart Connect.
  3. Log in to Chart Connect to securely access your patients’ records.
  4. Providers can authorize colleagues or staff members to view patient records.
  5. Records are accessible 24/7, including progress notes, testing results, medication orders and discharge summaries for up to three years after the patient’s stay at MU Health Care.

For security purposes, do not include any patient-identifying information in email correspondence.

MU Health Care is strongly committed to ensuring compliance with all applicable laws, regulations, and policies governing access to patient information. Each provider and their support staff who access electronic medical records must be uniquely identified. The National Provider Identifier (NPI) number has been established as the unique identifier and is required to obtain access to Chart Connect.
