Request Access


If you are a physician who refers patients to University Hospital, Children's Hospital, Columbia Regional Hospital, Ellis Fischel Cancer Center or our clinics, you may request access to view the medical record for your referred patients.

If you work with a referring physician, either as a care provider or as support staff, you may request access to view the electronic medical record for patients referred by your physician to University Hospital, Children's Hospital, Columbia Regional Hospital, Ellis Fischel Cancer Center or our clinics.

To request access, follow the two steps outlined below:
Step 1: Read, sign and date the "Access and Confidentiality Agreement for Physicians " or "Access and Confidentiality Agreement for Physician's Office Staff" and mail or fax to Medical Records.
Step 2: Complete all required information on the University of Missouri Health Care "Referring Physicians Online Access Form" and send electronically to Medical Records.

Step 1:

Print a copy of the "Access and Confidentiality Agreement for Physicians" or "Access and Confidentiality Agreement for Physician's Office Staff" which you can access below. Read, sign and mail or fax to Medical Records.

Physicians:

University of Missouri Health Care is strongly committed to ensuring compliance with all applicable laws, regulations and policies governing access to patient information. Here is the "Access and Confidentiality Agreement for Physicians." University of Missouri Health Care must have a signed and dated copy of this agreement in our files before access is granted to the electronic medical records for your patients.

Care Provider or Support Staff:

University of Missouri Health Care is strongly committed to ensuring compliance with all applicable laws, regulations and policies governing access to patient information. Here is the "Access and Confidentiality Agreement for Physician's Office Staff ". University of Missouri Health Care must have a copy of this agreement that is signed and dated by you and by your Sponsoring Physician in our files before access is granted to the electronic medical records.

Step 2:

Complete all required information on the University of Missouri Health Care "Referring Physicians Online Access Form." Here's the information you will need to complete the access form.

Physicians:

University of Missouri Health Care requires the National Provider Identifier (NPI) number to access patients' electronic medical records. The NPI number will be required of all providers by May 23, 2007, in all HIPAA standard transactions (Medicare and Medicaid reimbursed). University of Missouri Health Care must uniquely identify each provider, and their support staff, who access their patients' electronic medical records. The NPI uniquely identifies each provider and can be used to identify the support staff for that provider. University of Missouri Health Care is strongly committed to ensuring compliance with all applicable laws, regulations and policies governing access to patient information.

For security purposes, please do not include any patient-identifying information in your email correspondence. Fax and traditional mail are the preferred choices at this time.

Care Provider or Support Staff:

The access form requires you to enter the clinic NPI or your primary physician's NPI. Your physician is required to have access prior to you receiving access.

What happens next?

Upon receipt of your signed confidentiality agreement and processing of your access request, you will be contacted by a University of Missouri Health Care representative during regular work hours Monday through Friday 8:00 AM - 5:00 PM to establish your access to your patients' electronic medical records.
