Physicians who would like to make an appointment for a patient may call Referral Services at 1-800-877-7197, or they may call the clinic directly at (573) 882-3818 and speak to one of our staff physicians. To schedule a referral appointment, the following patient information will be required:

• full name
• date of birth
• Social Security number
• address and phone numbers for day and evening
• whether the patient has previously been treated at University Hospital and Clinics, either as an inpatient or outpatient.
