What should I expect at the time of my appointment? Clinic Appointment
Clinic Appointment
- For your appointment, please check in at the appropriate clinic area 15 minutes before your scheduled appointment time.
- Upon arrival you will be asked to verify key patient data and insurance information if applicable. Please present your photo ID and insurance card at this time.
- You will also be asked to pay any co-payment due, remaining deductibles or self- pay portions at this time.
- If you are unable to keep your scheduled appointment, we ask that you contact the clinic as soon as possible so they may use this time for another patient, if needed.
